Food Assistance Program (SNAP)
The Food Assistance Program helps people with low-income buy healthy food. If your household passes the Food Assistance Program’s eligibility rules, the amount of food assistance benefits you get depends on the number of people in your household and how much money is left after certain expenses are subtracted.
Food Assistance Program(SUNCAP)
The SUNCAP Program is a special Food Assistance Program for individuals who receive Supplemental Security Income (SSI). You may be eligible to receive food assistance benefits through the SUNCAP Program without any additional application, paperwork, or interviews. If you already receive food assistance benefits in the regular Food Assistance Program, you may be automatically put in the SUNCAP Program when you become SSI eligible. If your food assistance benefits will go down because of SUNCAP, you may choose to continue receiving your food assistance benefits under the regular Food Assistance Program.
Optional State Supplementation(OSS)
The Optional State Supplementation (OSS) Program provides monthly cash payments to indigent elderly or disabled individuals who live in special non-institutional, residential living facilities, including assisted living facilities, adult family care homes and mental health residential treatment facilities. To qualify for OSS, an individual must need assistance with the activities of daily living due to physical and/or mental conditions. The program provides a monthly check that supplements the individual’s income so they can pay the facility a provider rate established by the Department.
Home Care for Disabled Adults (HCDA)
This program provides case management services and a small financial subsidy to approved caregivers providing in-home care to adult persons (18 through 59 years of age) with disabilities who would otherwise be placed in nursing homes or institutions. Subsidy payments are intended to help offset the cost of housing, food, clothing, and incidentals, as well as those expenses related to medical, pharmaceutical, and dental services not covered by Medicare, Medicaid, or another form of insurance. A special pre-authorized reimbursed supplemental subsidy may also provide for assistance with the purchase of equipment, supplies, and/or services when regarded as essential to the health and well being of the individual.
Community Care for Disabled Adults (CCDA)
The Community Care for Disabled Adults (CCDA) program assists adults (18 through 59 years of age) who have a permanent physical or mental disability that restricts their ability to perform one or more activities of daily living and impedes their capacity to live independently. Through the provision of, or linkage to, in-home services. Services include but are not limited to: adult day care; case management; chore service; escort service; homemaker service; and personal care.

COBRA
The Consolidated Omnibus Budget Reconciliation Act (COBRA) requires most group health plans to provide a temporary continuation of group health coverage that otherwise might be terminated.
COBRA requires continuation coverage to be offered to covered employees, their spouses, their former spouses, and their dependent children when group health coverage would otherwise be lost due to certain specific events.

Housing Choice Voucher Program
The housing choice voucher program is the federal government’s major program for assisting very low-income families, the elderly, and the disabled to afford decent, safe, and sanitary housing in the private market.

Lifeline Assistance Program
The Lifeline Program provides a discount on phone service for qualifying low-income consumers. The Lifeline Program is available to eligible low-income consumers in every state, territory, commonwealth, and on Tribal lands.
Low Income Energy Assistance
The Low-Income Home Energy Assistance Program (LIHEAP) provides assistance to low-income individuals and families who are experiencing financial difficulty with paying their electric and gas utility bills.

Medicare
Medicare is the federal health insurance program for people who are 65 or older, certain younger people with disabilities, and people with End-Stage Renal Disease (permanent kidney failure requiring dialysis or a transplant, sometimes called ESRD).

Medicare Cost-Sharing Programs
State Medicaid programs pay Medicare costs for certain elderly and disabled persons with low incomes and very limited assets. There are several programs to help people pay their Medicare expenses: The Qualified Medicare Beneficiary Program (QMB); the Specified Low-income Medicare Beneficiary Program (SLMB); and the Qualifying Individual Program (QI).
If you already have Medicare Part A and think you qualify for either QMB, SLMB or QI assistance, you must file an application for Medicaid at a State, county, or local medical assistance office.

Medically Needy Program
Individuals who are not eligible for “full” Medicaid because their income or assets are over the Medicaid program limits may qualify for the Medically Needy program.

SHINE (Serving Health Insurance Needs of Elders)
SHINE is a free program offered by the Florida Department of Elder Affairs and your local Area Agency on Aging. Specially trained volunteers can assist you with your Medicare, Medicaid, and health insurance questions by providing one-on-one counseling and information. SHINE services are free, unbiased, and confidential.

Supplemental Security Income Benefits (SSI)
The Supplemental Security Income (SSI) program pays benefits to disabled adults and children who have limited income and resources. SSI benefits also are payable to people 65 and older without disabilities who meet the financial limits. The Foundation for Health Coverage Education (FHCE)
FHCE helps individuals learn their eligibility for public and private health coverage programs.
You qualify for hospice if:
You are eligible for Medicare Part A (Hospital Insurance) or Medicaid
Your doctor and the hospice medical director certify that you are terminally ill and probably have less than six months to live
You sign a statement choosing hospice care instead of routine Medicare covered benefits for your terminal illness
You receive care from a Medicare-approved hospice program
Some private pay insurance companies follow different eligibility guidelines and benefits offered can differ from insurer to insurer. Please contact your insurance provider about their hospice eligibility requirements.
Most hospices also offer comfort care for patients not yet eligible, contact your local hospice to ask about palliative care services.

Rebuilding Together
Rebuilding Together improves the homes and lives of homeowners in need (veterans, senior citizens, and the disabled with a household income of less than 80% of the median area average income). Typically clients are no longer able to work due to their age or disabilities and live on a small fixed-income. Referrals are made through community organizations, religious organizations, service groups, city officials, or by self-submission. Candidates complete an application, houses are assessed, and work scope determined. Qualified applicants are put on a waiting list for scheduling.

Lawyer Referral Service
The Florida Bar Lawyer Referral Service and Local Bar Association assist consumers in contacting an attorney. Under the Florida Bar Lawyer Referral Service, lawyers charge clients $25 (local bar-sponsored program charges range from $20 to $50) for the initial half-hour office consultation. The Bar has four specialty panels:

If your personal circumstances qualify you for referral to one of these specialty panels, you will receive a free initial 30-minute office consultation. After the first 30 minutes, fees are negotiable, based on the client’s ability to pay.

Senior Legal Helpline
The Senior Legal Helpline provides free legal advice and brief services by telephone to eligible Florida residents age 60 and older, for civil (not criminal) legal problems. Civil legal aid helps people access basic necessities such as healthcare, housing, government benefits, employment, and educational services. Eligible callers are scheduled for a free telephone consultation with an attorney or paralegal. Most callers will receive answers to their legal questions during the initial telephone appointment. Clients may also qualify for referrals to providers who offer free legal services in the client’s local communities.

Medicare beneficiaries can qualify for Extra Help with their Medicare prescription drug plan costs. To qualify for the Extra Help, a person must be receiving Medicare, have limited resources and income, and reside in one of the 50 States or the District of Columbia.

Program of All-Inclusive Care for the Elderly (PACE)
The Program of All-Inclusive Care for the Elderly (PACE) targets individuals who would otherwise qualify for Medicaid nursing home placement and provides them with a comprehensive array of home and community-based services at a cost less than nursing home care. Individuals who choose to enroll in PACE have both their medical and long-term care needs managed through a single provider.

American Association of Health and Disability (AAHD)
The AAHD Frederick J. Krause Scholarship on Health and Disability is awarded annually to a deserving student with a disability who is pursuing undergraduate/graduate studies (must be at least enrolled as a sophomore in college) in an accredited university who is pursuing studies related to the health and disability.
PAF’s Scholarships for Survivors
The purpose of our scholarship program is to provide support to individuals that are legal residents of the United States of America, under the age of 25, who have been diagnosed with or treated for cancer, a chronic, or life threatening debilitating disease within the past five years.

Driving Skills Evaluations/Clinical Driving Assessments
Professional driving assessments generally fall into two categories: driving skills evaluations and clinical driving assessments. A driving skill evaluation includes an in-car evaluation of your driving abilities and a recommendation regarding any further specialized drivers’ training. Clinical driving assessments are used to identify underlying medical causes of any driving performance deficits and offer ways to address them, so driving remains a safe option.

Developed by the University of Florida Institute for Mobility, Activity and Participation, this website is designed to provide information and resources on alternative forms of transportation, driving knowledge and skills, and links to national organizations that address transportation issues.
Miami-Dade Transit: Special Transportation Services
Special Transportation Service (STS) is available for people with a physical, mental or intellectual disability who cannot ride Metrobus, Metrorail or Metromover. Any resident whose disability prevents them from riding regular transit vehicles qualifies for STS. STS offers shared-ride, door-to-door travel in accessible vehicles throughout most of Miami-Dade County, in some parts of South Broward County, and in the Monroe County Upper to Middle Keys. STS operates 24 hours a day, seven days a week, including most holidays.

Vital Flight
Vital Flight serves those in need of medical treatment for a variety of reasons. The typical Vital Flight passenger is a patient seeking treatment or therapy that requires medical attention on a non-emergency basis. Vital Flight is not an air ambulance and all patients must be medically cleared by a physician to fly.
